Costantino Iadecola, MD, Weill Cornell Medical College, Cornell University, New York, NY, provides insights into the recently commissioned American Heart Association (AHA)/American Stroke Association (ASA) scientific statement on the neurovasculome. Recent progress in neurovascular biology has unveiled a complex interplay involving extracranial, intracranial, and meningeal vessels, as well as lymphatics and associated cells, collectively known as the neurovasculome. Prof. Iadecola highlights how the statement evaluated these advancements and their significance in the context of brain health and disease, leading to the redefinition of the concept of cerebral vasculature. The incorporation of technologies such as single-cell omics to explore the intricacies of cerebral vasculature has had a notable impact on our understanding of the neurovasculome, unveiling heterogeneous pathogenic mechanisms by which its disruption contributes to cognitive dysfunction. The statement identifies knowledge gaps and provides future directions for new diagnostic and therapeutic approaches to neurovascular and neurodegenerative disorders. This interview took place during the International Stroke Conference 2024 in Phoenix, AZ.
