Having spent six years in Japan, this actually rings true. The Japanese have a really difficult time changing and are inflexible to the extreme, especially bureaucrats. The expectation is that things are done, as they are always done, and it generally takes direct intervention to address things outside of the norm. Having said that, the reason things are as efficient as they are is because things don't change.

In the book, Ishido himself comes to deliver a message to the person Toranaga is posing as, so Blackthorn cries out that it's "terrible luck for a prince to deliver his own messages, " grabs the message out of Ishido's hands, and tosses it in to Toranaga. Even more audacious.

Think no one knew Toranaga made the switch due to the distraction, it was only Blackthorne and Mariko who noticed because they were observant. Guess they realized later why he acted like a fool

The original Shogun series handled this much better. The idea that Blackthorn had "gone mad" seems a lot more reasonable. Cultures around the world consider madness to be a gift from the gods. But saying "He is insulted because in his culture... " would probably get him stuck with more arrows than a porcupine.
