Its surprising to me that this doesn't happen more often even within the mid pack. If you assume two drivers will have lap times within .1 seconds of each other, the chance of tying should be at least .5% I would think. Multiple that by 20 drivers for 23 races and I feel like it would happen several times a season within the field and once every few years for pole.
