This reminds me of a time when I was playing Cities: Skylines and inadvertently placed a water tower too close to the industrial area and connected it to the rest of the water system.
Unaware of what I had done, before long I noticed all the buildings in the city had medical emergencies and dead bodies waiting for transport, and all the traffic on the streets were ambulances and hearses. I built two more hospitals to keep up with the demand and every single available ambulance was immediately dispatched, but was caught up in a traffic jam of 20 other ambulances. And ships were coming in one after another bringing in more people, who had to shove their way past the body piles to get to their new homes, which may still have bodies in them.
By the time I realized it was a single water tower that was the cause of so much anguish, thousands of people had died as a result.
