It looks like a by pass on the primary flow & return-was the valve open?

I’ve worked on similar set up but on a brand unknown only had a list of parts, which had to be ordered via bes I think as my supervisor ordered parts. That’ valve is likely to be issue as it has springs and if seized or blocked won’t open. We used to change loads until they removed them and believe it or not fitted another batch of 35 hiu units that no one has heard of🙁🙄 but the new brand do have a good technical team in place👍

Mate they are easy Well check the flow if the flow is above 200L Then we know that it's not communal issues
Hot water have 2 valve pm valve and tmv valve at the top chek the hot pipe before the tmv if it's cold then the PM valve is broken.
I did install 1000 of them any questions I'm happy to answer to help I'm specialist in HIU .
