Vietnam War veteran John Rambo finds peace in a Buddhist monastery in Thailand. He likes to work there and help with the housework. He earns money by participating in underground fights. His former commander, Colonel Samuel Trautman, received a request to investigate the situation in a remote area of war-torn Afghanistan. The US cannot deliver Stinger anti-aircraft missiles there to help the Mujahideen in their fight against Soviet troops. Trautman and CIA agent Griggs visit Rambo. Trautman asks him to join the operation, but John refuses.
Near the border, a Soviet helicopter destroys a Mujahideen caravan, Trautman is captured. Having failed to achieve cooperation, Colonel Zaitsev subjects Trautman to torture. Having learned from Griggs that it is impossible to rescue Trautman, Rambo asks to send him alone to Afghanistan. Griggs warns that the US will deny the very existence of Rambo.
Rambo arrives in Peshawar and meets with a guide named Musa Ghani, he knows where Trautman is being held. Rambo and Musa cross the border and come to a border village. At the council of field commanders, Soviet defector Yuri shows a plan of the fortress in which Trautman is being held. The Mujahideen refuse to help Rambo. Rambo and Musa enter the fortress under cover of darkness. They are joined by a boy, Hamid, whom they met in the village. John finds the Colonel's cell, but suddenly the alarm is raised. Rambo and his comrades engage in a fierce battle, they manage to escape through the sewers and hide in a cave, but Troutman remains captive.
Musa and Hamid return to the Mujahideen. Soviet troops are searching the area. Rambo climbs the cliff and enters the fortress. He leads Trautman and the Afghan prisoners out. Soviet troops return to base. Rambo hijacks the helicopter on which Colonel Zaitsev arrived. Soviet soldiers spray bullets on a helicopter as it takes off. Rambo lands the burning helicopter, says goodbye to the liberated Afghans and leaves with Trautman for the border. An enraged Zaitsev raises everyone to their feet. Rambo knocks out a helicopter, but Zaitsev drops a bomb from the second helicopter; Rambo barely manages to take refuge in a cave, the entrance to which was discovered by Trautman. Zaitsev lands a special forces detachment led by warrant officer Kaurov, who tortured Trautman, but Rambo manages to kill his pursuers.
Rambo and Trautman make it to the surface, Rambo defeats the mighty Kaurov. They are surrounded by a large Soviet detachment under the command of Zaitsev who has arrived in time. Rambo refuses and shoots back from the trench; the Mujahideen come to the rescue. Rambo captures a tank and shoots at Zaitsev's helicopter. The helicopter and the tank collide, Zaitsev dies, but Rambo remains alive.
The heroes say goodbye to the Mujahideen, Rambo gives Hamid his talisman. The Americans leave in a Soviet jeep for the Pakistani border. The end of the film is accompanied by credits: “The film is dedicated to the valiant people of Afghanistan.”
