I am wondering. It says ontop of noa2 an "A number". But most sources i see say that if the person hasnt visited the u.s. or if they havent been approved for work permit than should leave it blank, so what do i do?

If I am filing for my fiance's children, what do I do in the following parts of the I-134:

In Page 2, Part 2, Item #11: What is your relationship to the beneficiary? What should I put if it's my fiance's children? I put Fiance's Child. Should I modify it?

And, in Page 5, Part 3, Item # 9: It says "In Care Of Name (if any). Do I put the child's mother's name? I did put my fiance's name as she is the mother....I am sorry for the questions, I appreciate your help as I feel a bit confused but I think I do know the answers lol.
