Your logic has a major error. Each item should be a new item in the cart if there's at least one variation because when you are choosing variations, the latest choice is replacing the previous choices. For example, if you choose 1 small item first and then choose a medium item, i.e., 2 of the same item, the small item gets replaced with the medium and the cart then has 2 medium items instead of 1 small and 1 medium.
