I haven't watched this episode but I believe the context is that Lois made it so whenever Peter hears "extra!" his brain makes him.. Eat the closest person out. He had brought food to Lois' mother while she was watching the news, and the news said "extra" so I guess you can assume why they were both scarred, and why it showed a clip of Peter licking ice cream.
