He looks as if he is being contained in an institution, and don't forget how his personality seems to mirror that of Gray from Sprunki, with theories tied to everything being his hallucinations. My take is that Paul uses normal mode to cope with the trauma of his friends dying that we see in horror mode. I believe that those who are still alive visit Paul in the mental hospital, while William serves as the sleep paralysis demon trying to snap him back to reality. Oh yeah, and Amy's a drug addict.
