It's incredibly unlikely that handshake spot was a shoot considering it's an inversion of the handshake spot at the beginning of the match (Shawn offers his hand to Cena, and he rejects it). And Cena does sell the leg, very well in fact; he just stops after a while. The part where HBK is in his face is before that and it's worked. You can even see him mouth the word partner (in allusion to their tag team storyline, which lead up to this match)

Hot take: I like this match better than their rematch on Raw, however I still wish Shawn won the spinner belt at this time, even if it was for a short while.

I know he didn’t want to, but I think it would’ve helped his character have a layer of believability when entering championship matches (kinda like Sasha - She wins the belt, but loses it in her first defense).

This wasn't a shoot (I mean, the guy streaking was a shoot). Shawn and Cena were tag team partners at the time (were they the champs? I forget) Cena was trying to shake hands at the end like "Hey man we're still partners" and Shawn was like "Nah man not right now, I gotta blow off some steam"

Shawn wins the World Heavyweight Title at No Way Out and vs Royal Rumble winner Edge.
Meanwhile Batista pretends to be a Good Guy but his Feud with Rey on SmackDown has unleashed his Darker side, Flair rolls up Cena at the End of 2006 for his 17th World Title then Batista gets a Title Shot from his Friend at the Royal Rumble and everybody tells Ric Batista is playing him but Ric doesn't believe them because Dave is acting so Nice to him, They decide it's gonna be a Steel Cage and Batista Not only Wins but Destroys Ric Flair even after the Match prompting Cena to come down to make the save.
Then Cena vs Batista meet at WrestleMania 23.
