All medical software I've worked with track changes that are made (usually crossed out if everything is deleted/discontinued or by being labeled edited). In the USA we aren't allowed to take pictures of patient records due to HIPAA, but if it had been printed out (with reason of course) that would be okay as long as it stays in the hospital.

this happened to me. the doctor erased an entry which isn't allowed for obvious reasons. the hospital sided with her initially. thank god for my smartphone cause I knew the order was wrong.
