Mohgs shackle will reveal hidden paths just by using it in front of one or around one and the radius is pretty good on it. It will also activate those pesky flame towers that require you to hit them to make them go down.

Sometimes when there’s a rune arc in the secret room, there’s another door..

Noticed the area from that roof there other day, couldn't figure out how to get there.
Also, watch for empty bookshelves in that area. They seem to indicate a secret area

I literally have 249 hours in and it's still my first play through. But still I knew there was a way to that loot . I see it from the roof all the time

It's an illusion wall near debate palor boss room. And it's also a crawlspace as well

It was missed because it is a very poorly placed hidden wall. There was a freaking window there. It clearly isn't a doorway. This one doesn't scream "Whoa new hidden stuff" it just reminds me how poor the level designs are.

Yeah I found this..if anywhere there’s mages an magic related enemies I hit walls 😂…not sure if it’s the manor or the academy but bookcases with no books are usually hidden rooms too…there should of been something better in this room in my opinion, a cool spell or summon just because it’s a normal looking wall an well hidden

From what I saw you don't have the ladder or the 2nd grace in that room. Very large area.

Tip, go to the grace where you fought the wolf boss. Walk out to the big area immediately after, to the left there should be a staircase/shortcut to Renna. Don't take the bridge shortcut there is a staircase to the left that you can jump down to. Down the staircase is a door that leads to the courtyard you already have, up is fun!

Also, right BEFORE the wolf boss room. While you're walking up the staircase to approace his room, hook left. There is a secret passage and I think a weapon?

400+ hours is a lot, but I checked all of my games total playtime the other day and I have over 4, 000 hrs into Witcher 3, a lot more than I thought, and I knew it had to be a lot. And I haven't played that game in years.
