In the good old days the joke was that doctors had to have a bad hand writing. With everything being typed, the doctors had to find another way to make sure the patients doesn't understand them...

Don’t get me wrong, abbreviations are great. But sometimes one specialty has the abbreviation for one thing that a different specialty has for another. It gets confusing quickly. The Dr i worked for made up his own sometimes and we would get calls constantly from other offices asking what he was typing lol

I'm a medical coder now auditor (i spend all day reading medical records) and this is a little exaggerated but still very accurate😂

It's the way that I, as a medical student, have chosen to do the exact same thing then curse myself later when I make an abbreviation on the spot and can't understand it later.

As a medical record coder who codes for billing purposes and accuracy is necessary. We need clear explanations like the first note. The guy who documents like the 60 characters will get what is called a query and that MD will have to start all over with non abbreviations. Legally I can't code abbreviations and I have the hospitals revenue integrity department to back me.
