Here's my theory.
After watching this and the shooting scene a dozen times each, I conclude that Mrs. Kirkman is indeed shot. In the shooting scene, the bullet that hits the sniper's window does not cause him to shift, jerk, or move the rifle in any fashion. The scene right before shows the sniper fixing in on the President. The sniper was rushed to shoot knowing that the sound of Hannah's gunshot would prompt the secret service to human shield around the President. This most likely lead to a bad shot, instead of hitting the President, it hit the only significant character around him (the first lady). As many have already pointed out the figure on the gurney at 0:20 looks feminine. One that might say that's just how the blanket fell. This is a valid point. The strongest piece of evidence I have to support my theory is a few seconds before at 0:17 . When looking at the secret service agents, Mike is not present. There is no way in hell that Mike would leave President Kirkman's side. One might say that Mike went to protect the VP after Kirkman was shot. This proposition can be defeated by pointing out that the victim is still alive. If Kirkman was shot, Mike would have been leading the gurney into the operating room.
Side prediction:
Hannah is released by the end of the next episode.

Edit: I have a feeling someone will say that the agent at 0:17 is Mike. It is not . Compare Mike at 0:06 with the agent at 0:17 . I'm just going to say it, when the scenes go by as quickly as they do in this teaser, it is easy to say: "Black secret service agent; must be Mike!" I had to triple check myself, they are different agents.

My gut tells me it was the First Lady. She was right next to Kirkman I CANT WAIT THIS LONG THIS SHOW GIVES ME SO MUCH ANXIETY NOTHING EVER GETS RESOLVED. NOW HANNAH WHO HAS SLAVED HER ASS TO GET ALL THE RIGHT EVIDENCE IS GOING TO BE ARRESTED AND NOTHING WILL BE RESOLVED ONCE AGAIN GOD DAMN IT WHY IS THIS SHOW SO ADDICTIG

It's most likely Mcleish's wife. Agent Wells shot the window and the shooter jerked his shot to the side. Also what the hell is the reason behind keeping this series away for like 3 months lol ? They did this with Agents of Shield before too and it really kills momentum in a season when you have to wait so long.

Ok, I honestly think it was The First Lady, because there was foreshadowing in the episode when President Kirkman told her to take the kids and stay somewhere safe until the problem was solved and it was safe to come back to The White House. Also, Agent Hannah Wells, distracted the man who was aiming for Kirkman before he took the shot. His aim was probably messed up by that gunshot Wells took, and he ended up shooting the wrong person, The First Lady, instead of President Kirkman. Also Kirkman is the main character so the director probably would not make him be the one that was shot. President Kirkman and The First Lady were the first people you see in the beginning of the promo and then it goes to the next snippet of the upcoming episode in March so...And if you look closely at the body that the surgeons are wheeling in and putting on the table, you can kinda see a woman's breasts under the blue covering during surgery. I may be wrong, but that's what I think it was.This is my theory. I hope some people agree with me XD

You guys better see on the imdb. The one character who only plays for 10 eps in the list is the victim.

What a brutal episode, can't stop thinking about it! it actually received 9.7 on IMDB, more than any Homeland episode...great job!
I suspect the person shot would be Hookstraten...killing the first lady has no affect on storyline and would be too harsh, the show just started so you can't wipe her so early stages...but hookstraten's death can affect the story line because she seems honest - and she was getting in the way of peter so it would be a great choice to eliminate her so that peter could advance more easily. (even though it was not the intent of the sniper)
