The doctor's narrative report must include the following:
- WSIB claim number
- the diagnosis for your work related injury or illness
- a list of the medications you are currently taking
- response to medication(s) to date, including pain scores if available
- current functional abilities
- treatment goals and anticipated/ongoing benefits from the use of the medication(s)
- what side effects, if any, you have experienced as well as any action required as a result
- the dosage, strength and quantity of the medications(s) and expected medication changes/dosage increases, if applicable
- an estimate as to how long you will need to use this medication(s)
