I personally like the "contemplating suicide" theory.

"And when I miss you come and kiss me with a smile" -> He indulges in these thoughts; he feels lonely/without purpose without them and they bring him comfort.

"The only time I ever see her is when she's behind me in the mirror" -> He get's a more clear/stronger suicidal thoughts when he's thinking about himself/his future

"Even from a distance I can hear her" -> Even when he's feeling good or is distracted he still gets these thoughts

"I try to listen but her whispers make my ears hurt" -> He tries to understand why he feels like this and reason with the thoughts, but gets overcome by emotion

"And on our dates it's never daytime" -> The thoughts are stronger/clearer when he's alone, especailly lying in bed at night

"When she get's playful with a steak knife" -> Self-harm or a half-hearted/casual suicide attempt

"I need a break, maybe I'll take five; shake it up and medicate, maybe I'll take five" -> He takes antidepressants or uses drugs to cope, and contemplates using them to intentionally overdose

I love how when he says “maybe I’ll take five”, the piano part that comes in interrupts the perviously steady pulse in uneven groupings of 5 (try tapping your foot to the beat and it’ll line up every 5th time you tap your foot, hence “maybe I’ll take five”), and as a music nerd I am sooo living for that attention to detail and creativity

So, I've seen a lot of theories about this song. I've decided to put all of them here, very simplified

- Abusive relationship the singer can't get over, she's metaphorically haunting him
- Dead wife he hallucinates (he may or may not have killed her in the first place)
- Literally doing the sideways tango with a ghost
- has a cold/distant partner that at least partially exploits him for... amorous congress. and probably ghosts him a lot. metaphorical ghost-ness!
- "Flirting with death", contemplating suicide
