The first one could have easily been prevented by pitching the nose down and lowering flaps with other small adjustments, but the pilot was inexperienced and didn’t know that resulting in a crash. The Concorde wouldn’t have crashed if it wasn’t for the dc-10 debris on the runway.

In July 2000 Air France flight 4590 had a piece of metal on takeoff from Charles de Gaulle airport in France. Sadly the flight crashed into an airport hotel telling 113 people.

In most cases it is not the airline’s fault, it’s the company that makes the planes, for example if a 747’s systems fail, unless it’s because the airline did not keep it maintained, it’s likely because it was built Wrong or something like that. Also stuff just happens.
