Показать описание
It takes a team to manage a condition as complex as diabetes. Providing patients with appropriate educational resources can help them remember important information, enhance their ability to manage their health, increase their participation in informed decision-making, and ultimately improve patient outcomes. The University of Maryland Center for Diabetes and Endocrinology discusses the partnership with Johns Hopkins Health and how this FREE resource can benefit you, your patients, and the community as a whole.
In a new partnership with Johns Hopkins Health, the University of Maryland Medical Center will increase access to DPP for the prevention of type 2 diabetes by 20%, and will aim to expand access to DSMT for the management of diabetes by 25%. This partnership is made possible by a four-year $15 million grant awarded by the Health Services Cost Review Commission (HSCRC) to increase participation in DPP activities for residents of Baltimore City and increase enrollment in diabetes management services throughout the state.
