I've heard a story about this before. A woman was complaining about unspecified abdominal pain. Workups were unrevealing. Somewhere down the line, a worm doctor took over the case. Maybe a worm was causing it, I'm not sure. But the clue to the diagnosis SMA was the patient said that the pain felt a lot better when she was doing yoga, specifically that downward facing dog pose.
The problem I hear about definitively diagnosing this is it takes a very specific angiography.

I was diagnosed with SMAS in March 2020 (right when lockdown for COVID) I was hospitalized for a month and sent home with a surgically placed Peg/J tube. I had an NG Tube for a week. This syndrome is horrifying and scary. I weighed 85 lbs (at 5”8’) and was slowly dying until I was diagnosed and treated for weight gain. I went 100+ days without eating or drinking (obly fed through tube)

Now after 2 years it’s been a year since I got my tube taken out and I’ve gained a lot of weight (I weigh 160 lbs) and live a healthy life.

It took almost dying from malnutrition twice over a year and 8 months to discover I am suffering from SMAS. At my lowest weight of 108lbs, I was put on a NJ feeding tube, but still we were unsure what was causing my wasting away. What delayed the diagnosis? Marijuana is legal in my state and every doctor wanted to write off my symptoms as CHS. Thankfully a doctor 6 hours away from my home was too smart to fall for the easiest answer. While I did quit smoking marijuana, it was a CT scan that confirmed the SMAS diagnosis. Quitting marijuana was never going to be enough to save my life. I have since been put on Cyproheptadine, in an attempt to gain an appetite. I was told I need to gain 20lbs. I am struggling to eat more than 1, 500 calories a day, when I personally need a minimum of 2, 000 calories a day to recover. I am trying very hard to get through this for the sake of my young daughter. As I am very depressed after my diagnosis, I appreciated the humor in this educational video.

At the end of your video - do you have a recommendation for which type of surgery is best? It seems like there are multiple options with multiple opinions on which is best.
