Heyy cory, you probably won’t get to this, BUT In Shinkansen 0, when the train announces a “personal injury involving a young boy and a young woman, ” it heavily implies that Naoki and his sister died by being struck by the train—possibly while trying to board or cross tracks, outside of the train, their spirits lie where they died, which was close/inside the train. The main protagonist, you, helped to release their spirit. The game leaves it vague, but the use of that term points toward a tragic accident involving the railway. Hope this helps.

My takeaway on this game after watching you (yes YOU, Cory) play this amazing game:

You board the bullet train. You pass out while on the train from a deep vain blood clot (diagnosed by the doctor in Ending 2.) You play as your spirit trying to get back into your human body: the gameplay of looping on the train is you trying to find a way to get back into your body and wake up. The train crashes into a boy and his sister while you’re still unconscious, killing them both. You meet both the boy’s and the sister’s spirits while you’re stuck looping. I think that they noticed another spirit (you) on the train and disguised themselves as passengers to try to help you stay alive, explaining why they are sitting at row 4, as they are dead, and also explaining why you can only interact and talk with those two, as they are also spirits, with exception for the old man later in the gameplay, also explaining why the old man states that you are not “human” because you are a spirit. This might also suggest that the old man is also a spirit, and might be dead, since you can interact with him, and since he stated that no one responded to him on the train except you.

At the end of the game, you figure out the solution to end the looping after talking with the sister. Both of you conclude that running different directions on the train at the same time will end the looping, but before you do, you get to either choose to run toward the green car or toward the regular car. I think the green car is the afterlife and the regular car is your human body. If you choose to run toward the green car, you end the loop by going to heaven, but this also means you die with the boy and his sister (Ending 1). However, if you choose to run toward the regular car, you escape death as you get chased by the grim reaper. You then meet with the boy’s and his sister’s spirit to say one final goodbye. I think the boy gets a little attached to you after what you guys have been through, so he asks if you want to come with them, in other words, if you want to die with them, however, the sister disagrees and says “It’s not your time yet” since you just escaped death. Finally, you enter the regular car (your body) and wake up from being unconscious, overhearing that a woman and her brother (the two spirits you met while unconscious) were involved in “the accident” (the train hitting them). I also think the last scene of the boy and his sister was either to show them going to heaven or to confirm that they were the ones that got hit from the bullet train since they were walking on train tracks (Ending 2).

Shinkansen Symbolism
I feel like the Shinkansen is some sort of symbolism for dead people to pass onto the afterlife. We see trees when the train stops at the destination just like the cutscene at the end with the boy and sister, and is returning them to the afterlife with the tracks to symbolize how they died.

Protagonist
Since the protaganist was also at the train, it could possibly mean the protagonist was "dead" and was being transported to the afterlife aswell. This is true by the cutscene with the doctor and lady.

Sister Theory
Another theory I have is the sister could've possibly be still conscious but kinda dead at the same time??? Since the sister didn't wake up earlier then suddenly woke up as soon as the train stops, it means that she died and is being transported to the afterlife with her brother. Since her brother was there first at the destination meaning he died first and was waiting for his sister in the afterlife.

Sleeping symbolism
We first meet the old man right infront of the door and possibly trying to go to the afterlife (because he has lived a long life) but ends up sleeping and he even knows about it, he goes to sleep in the restroom along with many other people sleeping aswell (except for the people in the green car). This could possibly mean sleeping symbolizes "not ready" to go to the afterlife or they just died.

Monster thingy
I have absolutely no idea whatsoever about the monster thing chasing the protagonist throughout the Shinkansen. But, i think it's meant to symbolize the reaper trying to take the protagonist's soul to the afterlife since we didn't see that when we went to the green car.

Car meaning
The different cars could also mean different things, the green car is meant to represent going to the afterlife, while the normal car represents just dying or returning back to reality. Also, another thing I kinda missed earlier is the fact whenever we stop the train, it means we picked up another dead person(The sister for example) or whenever we start the train we drop them off at their destination.
