Do you have a child or student preparing to transition to high school this fall? In Pennsylvania, secondary transition planning starts at age 14 for students with disabilities. In fact, there is an entire section dedicated to it in the student's individualized education program (IEP)! But what does this section entail, and where do you begin? In this session, we'll explore the Transition Grid in the IEP to help prepare your family & student for a successful secondary transition process.
Participants will learn about:
-The three sections of the transition grid in the IEP
-The information that should be included in each section of the transition grid
-Examples of transition “Goals,” “Activities” and “Services”
