That people is the result of some serious traction and power!

The only reason the bolt bent is because the axle moved forward.. possibility of it being loose the bent bolt is only to help hold the axle in place when you tighten the axle nut I've seen this on my own bike before and I didn't tighten it correctly... So I'd bet it was loose or it needs a higher torque to keep it in place..
