Lumbar Spine Transverse Process Fracture - Everything You Need To Know - Dr. Nabil Ebraheim

Dr. Ebraheim’s educational animated video describes Fracture of the Lumbar Transverse Process.
Is it benign? The transverse processes are covered by thick muscles. Fracture of the transverse process is rare. It may occur as an avulsion fracture due to strong contracture of the muscles. The injury may also be due to sudden extreme twisting, side bending or by direct impact to the process itself.
The injury will not cause instability of the spine or spinal cord injury. Transverse process fractures are usually treated with a corset or brace, but not by surgery. Associated injuries may occur to the abdominal viscera, retroperitoneum and the pelvis.
This is a minor injury caused by a major force. There is a frequent association with intra-abdominal injuries. If you can see the fracture on x-ray, look for other injuries.
The fracture may be missed on a routine x-ray. CT scan is better at defining the fracture.
Here is an example of a patient with a benign looking transverse process fracture with an associated significant injury to the back.
An L% transverse process fracture in the presence of a pelvic fracture is a predictor of pelvic fracture instability. Its presence should alert the clinician to this possibility. For example, an L% transverse process fracture may be a sign of an unstable pelvis. It occurs due to an avulsion of the iliolumbar ligament.

I was in a bullriding wreck and had three transverse process fractures in my L1 L2 and L3. I've been through labor and kicked in the shin by a horse and neither one of those compared to when this happened. It's the worst pain you could ever imagine.

Also, known as a Scotty dog break. My lower lumbar was broken in several places and damage to my neck. My last back surgery, an ALIF and PLIF was done to L-2 to S-1. Five vertebrae were fused with pedicle screws and rods, and the disc were removed. I am having C-6 and C-7 fused to help prevent further atrophy of my hands. I can now walk with some permanent nerve damage. The unrelenting chronic pain makes it hard to have joy in life. I have an itrathecal pain pump with high dosage of Fentanyl and Bupivacaine that has kept me going. I can no longer work and get very little sleep. I do some time feel sorry for myself and when I think of others much worse off than me makes me grateful for my Doctors and my new wife. I did the trial for Nevro HFX spine stimulator and have high hopes for permanent placement.

I was in a horseback riding accident and sustained an L4 compression fracture and a transverse process fracture in November. It’s amazing how tough yet fragile the human body is.

Thanks for producing this video.  I recently was crushed between my horse and a low tree limb, resulting in rib and transverse process fractures. I'm relieved to see that healing will come with time and patience.

decades ago, I suffered a fracture of the L5 right Transverse Process, as well as multiple pelvic fractures. The injuries went undiagnosed for 6 weeks (L5) and 35 years (pelvic).

I fractured l2 to l5 completely broken off playing football, got a hard knee in the back,
Hurt like crazy for 3 weeks couldn't sit down because it was causing a nerve to pinch and could only sleep in a certain position,
But healing for me anyways was quite quick and I've never had any problem with my back since,

Anyone who's had this... how long did it take you to recover from this fracture?

All 5 of mine 7 years ago. Took a couple month to not be dying of pain. Got better for a few years but I’ve been having knee and hip pain and all over soreness. Comes and goes so I wonder if I gave nerve damage surfacing?

In L2 L3 and L4 Transfer Processes, non-discrete break lines were formed due to falling from stairs. It took 60 days for me to recover. It's been 1 year and 4 months today. Sometimes I feel pain in that area when running at a light pace. Is there a possibility that the areas where the edema joins will melt again? I don't like going to the doctor, my pain is not severe, it's very mild...
