The aircraft involved in the accident was a 29-year-old McDonnell Douglas DC-8-61, manufacturer serial number 45942, line number 349, registration N27UA, operated by Fine Air, with 48, 825 total airframe hours and 41, 688 cycles


There were three crew members and a security guard on board. The
captain, 42-year-old Dale Patrick "Pat" Thompson, had been with Fine Air
since 1993. He had a total of 12, 154 hours of flying time, including
2, 522 hours as a DC-8 captain at Fine Air. The first officer, Steven
Petrosky, aged 26, hired on 15 August 1994, had a total of 2, 641 hours
of flying time, of which 1, 592 hours were with Fine Air in DC-8s and
logged 614 hours as first officer and 978 hours as a flight engineer,
all in the DC-8. The flight engineer, Glen Millington, aged 35, had
joined Fine Air in 1996. He had logged a total of 1, 570 flight hours,
including 683 hours as a DC-8 flight engineer at Fine Air. The security guard on board was 32-year-old Enrique Soto

The engine didn’t fail, there wasn’t enough air entering the engine due to the high angle of attack and that caused the engine to serge.

That's not the pilots fault. They weren't told the correct weight, they had an extra 5, 000 pounds to thr Back of the plane and they were never told that. It was the Fine air managers fault. He didn't want to take responsibility.

The reason of the crash, determined bu the NTSB was human and pilot error. The cargo was heavily influencing the Center of Gravity to go far out of the safe area. The pilots didn’t get the proper cargo configuration as that was the original plan, and then it was rearranged without the crew knowing. The pilots set the trim to 2.4 which is what was indicated by what they were told. But that was wrong and it was unfixable once they did an emergency re-adjustment. The part where he says Engine Failure, is not true. The reason why flames were shooting out the back was cause of thr flow of oxygen was interrupted by the high pitch, causing some fuel that was ignited, not to get extinguished naturally until it was through the outtake.

There was no improper trim? They set the correct trim for the cargo they thought was loaded correctly, they just changed it mid take off to get the nose down. There was also no engine failure, it was too much fuel getting in the engine, and not enough oxygen. The flames shot out to burn that extra fuel being poured in the engines. But the unprofessional cargo loading was correct.
