Plugs are multiple hairs growing closely together due to old hair transplant technique called (MUT) Multiple Unit Transplantation, where 5-20 hair grafts placed into each site. The down sides of MUT are: 1) "Donut" growth pattern occurs when the hair grafts in the central region die off when there is not enough Oxygen diffuses into the center region to feed the grafts during the early healing process before the underlying blood supply mature to nurture the hair roots. There are many wasteful hair grafts loss due to MUT because of the "Donut effect" 2) Plugs occurs when there many hair grafts (5-20 grafts) placed into one site, leading to hair growth in patch, multiple hair growing closely to each, clumping together, giving unsightly look or "Dolls like" look. There are three techniques to fix hair PLUGS. The three techniques involve: excising the Hair Plugs by the linear technique, building the transitional zone in front of the Plugs and FUE or Larger punch to thin out the Plugs. Though the Linear excision method may be able to remove some Plugs, but the trauma, linear scar and numbness make this technique not very desirable. As a matter of fact, the patient may appear worst after the surgery, because the scar is very large and noticeable. The other down side of Linear excision are: very painful during recover, high chance of bleeding and higher chance of infection. Building the transitional zone by FUT only while not thinning out the hair Plugs will not work well because the newly built transitional zone alone will not grow strong enough to hide the harsh Plugs. The best method involves the FUE & larger punches to thin out old Plugs and then build a small transitional zone in front of the Plugs with a feathering pattern to recreate a very natural & dense hairline. Sometime, more than one session is needed to achieve the best and very natural result. I have helped many patients who suffered with noticeable and unsightly PLUGS to look more natural and dense.
