I'm not 100% on this but I'm pretty sure the one thing that really separates them from other bikes is the lack of a freewheel, where the back wheel can move forward independent of the cranks. So, if you pedal forward, the wheel moves that amount forward, but will stop when you stop pedaling. If you stop pedaling, the tire stops. If you pedal backwards, the wheel goes backwards. I honestly find it kinda stupid but w/e people like what they like.
