What you've completely missed about Donatella's surgery, if I'm to assume which pictures you're showing are supposed to be more recent (because you don't clarify), she appears to have had her nose fixed. For years it looked like she had no bridge! It was flat against her face and that is what has looked the strangest. But now she appears to have had the nose bridge rebuilt, and it looks much more normal.
