She's literally giving birth? People scream when they do that. It was a good display of her acting range and it also did well to draw back to the elongated scenes of suspense and discomfort. My opinion obviously but I think it was really well executed

Say what you want about this film, but the scene where Cecilia used a chicken's blood to fake a miscarriage in an attempt to get herself out of the convent was an interesting moment for her character. It proved that although she was incredibly distressed by her circumstances, she also was capable of thinking on her feet and making resourceful decisions. We don't see that too often in horror, so props to the writers for following through with that.
