You shouldn't be using the NYHA classification to stratify SOB unless you have confirmed that is a cardiac etiology.

And Hypertensive heart disease is same as Ischemic heart disease??

Means hypertension is responsible for Hypertrophy and thus the remodeling and Oxyden demand, supply imbalance
