That happened to me. I was training a beginner and we were sparring. I told him to relax and we're just tapping, not punching. He threw a right hay maker and a left hay maker. They both landed on my jaw.

For the next 3 minutes I gave him enough Charlie horses to remember me for the rest of the month.
