The Milk Grotto is Behind the Nativity Church and Next door to Blessings Gift Shop and the Olive Wood Factory in Bethlehem. The Holy Family found refuge during the Massacre of the Innocents before they could flee to Egypt. A "drop of milk" of the Virgin Mary fell on a rock in the cave changing its color to pure white. On this rock, a church is built today called the Milk Grotto, where many Christians and Muslims believe that scrapings from the stones in the grotto boost the quantity of a mother's milk and enhance fertility.

The original Church was built during the fifth century by St. Paula, who lived in Bethlehem. The Church was dedicated to the Virgin Mary and later dedicated to St. Paula. During the fourteenth century, the Church was known as the Church of St. Nicholas. The site is currently managed by the Franciscans, who added the present building atop the cave in 1872 and expanded it in 2000. Some of the original church mosaics and traces of the original wall's remains are still visible today. Both Christians and Muslims believe that scrapings from the stones in the cave boost the quantity of a mother's milk and enhance fertility.
