A preliminary report by lawmakers investigating the July assassination attempt of Republican nominee Donald Trump says it happened because of a "lack of planning and coordination between the United States Secret Service and its law enforcement partners."
The report goes on to say that the "tragic and shocking events of July 13 were preventable and should not have happened."
