They died spiritually. This is why we as humans are born spiritually dead. This is why God sent His Son to die for all the sins of the world. This is why we must become Born-Again.

The death he meant was spiritual death. I.e separation from God. Which is to say that men would no longer have direct communion with him. In order for them to access him they would have to make SACRIFICES.
When Jesus died he repaired that relationship so that anyone who believes may not perish but have eternal life. Jn 3:16

- I always thought of it as, the day you eat from the tree, you will die, not literally then and there, but because you ate of the tree, you are now capable of doing what you weren't able to do before which is die. They now are going to get old and perish. They can no longer avoid dying. So, because you ate from the tree, you're gonna die....no one said instantly...
-EDIT : As far as a day...That specific passage, I believe, is just symbolic For how time affects The Father....He is not bound by Time, he can see the End from The Beginning...for us, 24hrs is a day but if we were seated with The Lord, 20 thousands years could have passed in our 24hrs. How could you tell Time in a place where Time does not exist?

They were made to live forever… the moment that fruit was eaten, their days were numbered.

Genesis 2:17: “ ‘But of the tree of the knowledge of good and evil you shall not eat, for in the day that you eat of it you shall surely die’ ” (emphasis added). The Hebrew text uses an idiomatic construction literally translated “dying you shall die.”
Meaning they will die spiritually first and then later on die physically. This puts the emphasis on “Jesus Christ” we need a savior to take the judgment for Adams original sin that is passed down from father to father. Since Jesus was mankind’s sacrifice for sin by dying on the cross. Sin will no longer be the issue for those who believe in Christ.

Honestly, my favorite take from the Book of Genesis, and probably the most demonstrably true, is "humans can't follow simple instructions." God had ONE rule for them. "See that tree? Don't eat the fruit, it'll kill you." And what did they do? They ate the fruit. People can't follow simple instructions.
