👻 Spooky hospital story! As a new doctor in a 200-year-old hospital, I encountered some eerie moments on the night shift. Imagine being in pitch darkness, with only your stethoscope for company, and then... a cold gust, a gurgling exhale, and a waste bin that closes by itself. Still gives me chills!

Confirming the deaths of patients is an essential skill for doctors, and something I've had to do countless times!

How do we do it?

The Academy of Medical Royal Colleges (AoMRC) says death is the permanent loss of the core functions that define human life. This means death happens when someone can no longer be conscious or breathe on their own, and these abilities cannot come back.

Although the brain ultimately controls life, death can be diagnosed by looking at signs in the heart, brain, or body that show these functions are lost forever.

In addition to listening to the chest to confirm there are no heart or breathing sounds, I also check for:

No reaction to being spoken to.
No pulse on the carotid artery.
No eye reflexes.
No muscle response to a shoulder squeeze.
No other signs of life.

When I was in Landstuhl after hitting an IED in Afghanistan my roommate was a SSG that had been shot 3 times and was in a medically induced coma. The second night i woke up to a loud bump, looked over and saw him trying to stand up, I called for the nurse, but the SSG looked over said “wait am I dead?” He collapsed, and was pronounced dead 2 hours later. The doctor said his body just gave up on him, and when I asked about him getting out of bed I was told that it was just delirium from waking up from his coma.
