According to Roger McIntyre who has researched mixed states extensively the most important symptoms are the 4 A’s Anger, Agitation, Anxiety, and Attention problems.

These symptoms overlap with GAD, ADHD, PTSD, and borderline personality disorder. This make it nearly impossible to distinguish the diagnosis based on DSM criteria alone.

With respect to these symptoms, are we make a comparison to the patient's baseline symptoms or just looking at overall level of symptom expression? For instance, if a patient is already known to have ADHD, would we be looking for greater-than-normal attentional disturbance for this individual or not?
