The slice of cheese representing the patient is incomplete, you can educate the patient but what if the patient is shown the bottle/packet of pills t see the name of the drug - what if the patient not only sees it but is allowed to write about the error in her/his clinical notes? Is the error and its root cause and resolution transferable in terms of remedy/change ins practice to every other hospital, like it would in the aviation industry? If not, why not? What if the patient dies ?- Are the Coroner's recommendtions applied not only to that hospital but every other one and are such recommendations imparted to every patient welfare and advocacy group in the State? If not, why not?
