In Star Wars why is it that the most powerful of force users refrain from choking each other or throwing one another across the battle field?

TL;DR: Jedi and Sith put up their defenses against force powers just like they defend against lightsaber attacks. Its all mental fortitude and focus, until they lose it, then it gets taken advantage of.
Also, Jedi and Sith seem to only engage in lightsaber combat once they've determined they're of an equal force level and can't take each other out immediately using the force... This is why Darth Vader, in his full form, can pretty much crush literally anybody, and probably only engaged in lightsaber fights with Luke because he didn't actually want to kill him, but turn him.
