Redundant PSVs may be utilized but I would think that's not a common choice for operational reasons. My thoughts are that a PSV with the same probability of failure on demand would reduce the likelihood of an event by a factor of less than 10, provided that maintenance maintains and properly tests the function of the units each year as part of their plantwide maintenance program. The latent mechanical failure of a PSV when demanded may be hidden. Additionally it is not completely independent. Safety systems work best when they operate on different fundamental physics principles. In the provided example there is likely a typical spring-loaded pressure relief valve system for the first layer, and on the second layer there is the oil-filled wheatstone bridge transducer used by modern day pressure gauges which is the industry standard technology for pressure gauges.

Side note: Many electronic instrumentation devices contain built-in functional check, so that the devices run a self-checking routine to ensure operation. In addition to this, calibration of the involved devices is a key. Theoretical LOPA analysis only takes one so far, but knowing how these choices will affect how operations manages the day-to-day, month-to-month tasks is what will bring the most value to the organization.
