Supermassive Black Holes are not made from lots of intermediate Black Holes. It is far more likely that they are gas clouds so large and massive that they were able to collapse and bypass the star stage entirely. Otherwise we are talking about thousands of black hole mergers which doesn't make much sense- especially because we don't see that happening anywhere in the past.
