there is no set up and you can do it with a borrowed deck.

you dont need to set up at all.. the card is a random choice it is controlled and he messes with the deck and sets it up while his back is turned

This is how I think he did: he controlled the card to anywhere he liked, when he asks to pick a number and he turns back, he moves a small pile of card six in this case with the selected card in the fifth position. The trick is pretty good, but it would be way cooler if you could do this without turning back, which is always a pretty sloppy move to do in front of an audience.
