Learn how to create online mobile-friendly forms and surveys using Microsoft Forms so users can fill in the form online and you will get the results back in an Excel spreadsheet so you can track and analyze your data. MS Forms allows you to send users a link to fill in your form or survey online, or you can embed the form on a webpage to collect responses. Microsoft Forms is available to Microsoft 365 work, school, and personal subscribers, so users with a Microsoft account who have access to the online office apps can use Microsoft Forms.

With a Personal M365 account, you can initiate the form or survey from the Forms app itself and then export the responses to Excel at any time. Then see how using M365 for work or school will allow you to initiate your form from the Excel online app, which dynamically links it for real-time updates as responses to your form or survey come in.

Ms. Smith. Thank you so much for your tutorials on YouTube. I'm hoping you can direct me here. I work at a hospital as a phone operator It is the duty of whomever is on night shift to make what we call a pass down sheet. Briefly, the pass down sheet tells the telephone operators which nurses or doctors are on call for a given department.
The way we do it now, is to fill in a Word document and we have to enter each nurse's or doctor's name, and in some cases their phone number(s). As you can imagine, this is time consuming.
I want to create a new form that will allow the phone operator to choose the given name from a drop down list, and auto populate the phone number. Here's the catch. Some departments will have multiple employees on duty. When it comes to those, I want to create ONE list and repeat it on multiple lines.
For example in my department--the phone operators--I have to fill in a day shift person, a night shift person and do this for two operators each.
Can I create a master list for each department and just link them into the appropriate parts of the form?

I think I saw the website embed link, but is it as simple as taking that code and dumping it into your object in your cms?

I am new to ms forms, but it's not clear whether or not I can create an application form where I need to collect personal data of each candidature and other information. If possible how can I add buttons, text fields, upload field, drop-down item and so on? I'm not interested in creating a survey form. Please let me know. Thanks

I am creating a form along with you. When I click the three dots mine does not say "drop down etc" Mine says "subtitle, Restrictions and Add Branching"

This is a duplicate question. If you were making a self fillable form, and you have a question which you want the staff to be able to choose more than one item in the drop down list; how would you do it?
