Civil war wasn't Tony's fault that just was to cool the situation Tony wanted everyone to be together and that was the only possible way but still I don't blame the captain because only Tony was the one who is afraid of the future captain was unaware about all the threats outside of earth

The thing people always forget is that Tony didn't propose the Sokovia accords the government of the world (UN) did, so even if Tony stand against the accords they will still force the heros to either sign the accords or retire from being superhero.

Most of these mistakes had a very dire strike on his Emotions and thoughts that resulted in him making such blatant mistakes.

Happy was gravely injured, and his best friend was in such a state that Tony was so angered he challenged a Terrorist group that had no qualms about killing hundreds without mercy.

Agreeing to the Sokovia Accords ties directly to the Ultron incident, Tony was placed between a rock and a hard place, he believes the deaths on Sokovia were all his fault since he was the one that pushed for the creation of Ultron. Tony believed that someone has to keep watch on them to keep them from taking too far, he was too stressed to realize how much the Government wanted to grab hold of them all.

The creation of Ultron stemmed from his vision of the death of everyone, it lead him to want to protect everyone by building a suit around the world. It's basically another PTSD triggered decision.

No 1 is a misunderstanding, tony wanted to create something good but ultron turned out to be like that because of the mindstone, and couldn't have done much .

Tony was scared attack in New York on 2012.
That's why he created Ultron he tell that in Endgame.About Civil war UN proposed that and if he didn't agree that he also will have to live like Cap. He don't want that since he is a billionaire and he needed to just live with pepper happily.
