Here you make a choice you can let those people who help you died and live with the guilt or you can save them even though you put your life on the line. He might not have been able to save Sam( If he died from these CIA guys) but he did save Cade even though they our on the run

I just realized that Sam most likely died after the CIA hunted him and Optimus down, so when Optimus saw it happening a second time with more innocent people he couodn't let it go.
