Setting: Hospital
Nurse: You are taking care of a 60-year-old who has an AKI (acute kidney injury, also known as acute renal failure/ARF). He /she has been given fluids and has blood tests.
Task:
• Find out how the patient is feeling
• Find out the patient’s reason for wanting to be discharged.
• Explain why immediate discharge is not advisable (e.g., more fluid required: one more day minimum; awaiting blood results; importance of monitoring AKI/acute kidney injury; etc.).
• Emphasize that self-discharge is not recommended (e.g., worsening condition, possible complication, bladder damage, kidney failure, etc.). Explore alternative care arrangements for his/her spouse (family, friends, etc.).
• Reassure patient about discharge (e.g., as soon as possible, lengthy stay unlikely, etc.). Find out if patient has any other concerns.
• Stress importance of self-care to avoid future admission to hospital (e.g., good hydration, balanced diet, respite care for spouse, avoiding medications such as ibuprofen and other NSAIDs/nonsteroidal anti-inflammatory drugs, etc.)
