The stairs are uneven. The step people are tripping on is measured incorrectly and is not the same height as the other stairs.

Explanation if all steps are measured to be evenly and one step is measure shorter/ taller with or without the yellow add one. Thus it becomes a trip/fall hazzard. The added yellow pieces might also not be the same height when added to the stairs, making 1 step incorrect in height.

Your brain goes into auto pilot when it expects a certain pattern to raise your feet, going up/down stairs. where all the same height 1 being off messes with the assumed height the brain had for you. This causes people to trip and fall.
Example step is measured 7 7 7 7 8 7 7 7 you will trip on 8 going up or down simply because of the assumed pattern of stepping up.

It’s because it’s not at the same; that step is a lot lower then the rest so it brings a deeper step to the point where the tip of your toes touch & you lose balance
