IOSH MS- Module 6- (Investigating Accidents and Incidents)

In first part of course we have focus to control accidents and incidents but still accidents and incidents occur.

So first major reason is to investigate accident due to prevent it from happening again.

b. Notifying to senior management
c. Notifying to insurance company
d. Determining cost of an accident

There are three causes of accident
1. Immediate cause
2. Underlying cause
3. Root cause

C. How do you carry out an investigation?

Steps of accident investigation:-

1. Provide first or medical treatment to victim
2. Barricade/ Preserve the accident area
3. Report the accident or incident to:-

a. Senior management
b. Enforcing authority
c. Injured person family
d. Owner of assets
e. Insurance company
f. HR Safety Department
g. Safety Representatives

4. Assemble the investigation team:-

a. Senior management with authority
b. Supervisor or manager from work location
c. HSE Expert
d. Technical Expert
e. Employee’s representative
f. Person involved in accident if possible

5. Start Investigation through:-

a. Physical visit at accident area
b. Interview with people
c. Checking documents

6. Handling external relations
