The cause of the rope twisting up like that is when the rope is being reeved on to the hook... they have not taken the twist out of the rope when running rope through the block. Common error and looks like as they let that much rope out the block starts to rotate, twist up. They shouldn't be lifting like that, that rope will be warped, memory in the rope, and as he lifts the rope becomes tight at the knot and snap that rope and the load are goners. Shock load crane, potentially kill someone.. cause a lot of damage. Hopefully this problem was addressed A.S.A.P. safety of the workers and equipment before schedules.

First thing i noticed was that his block waa twisted! That alone is a no no! Thats comes from running the hoist cable against a steel surface

I worked offshore as an Iron Worker. We are serious Riggers and when we received a shipment we didn't hook on from the boat. The deck hands did that and they were super fast. With twelve foot swells it goes up and down extremely fast. You can and possibly will loose a finger or a hand on a split second. I blame a bad Operator on that shit and the deck hands are slower than an old turtle.
