For those who said Nahida reads minds through the Akasha Terminal, go play the Archon Quests and Nahida's Story Quest if you don't want spoilers. Otherwise, read on:

It's actually explained in both aforementioned quests that:

1. The first thing Nahida did after saving Sumeru was shutting down the Akasha terminal once and for all.

2. Nahida's ability to read mind is innate and works on just about anyone. She even used it on Dottore, and, in case you have forgotten, random animals and Fischl and co. while they were on vacation at the Golden Apple Acchipelago during version 2.8.

From (1) and (2), it's likely that Nahida's power to read minds was the basis for the creation of the Akasha terminal, not that the gadget enables her to do so. Think about it, the Akasha Terminal collectively, yet at the same time, selectively allowed every Sumerian the ability to read someone else's thoughts, they just don't get to know to whom the thoughts they read originally belonged.

As for the reason why it currently doesn't work on people outside of Sumeru, the logical explanation is probably it takes too much time, effort, and probably even storage to implement it on that scale. Like, the update was nearly 3gb, and Nahida can still only read Sumerians' mind, can our device handle the whole of Teyvat?

The in-universe explanation may be what Nahida said in her quest: "I try not to use it because it's an invasion of people's privacy!"

She can only read the minds of the people in Sumeru, and they have to be npc’s that you can talk to (minus katheryn). No they do not need to have an akasha terminal since she can read the minds of people in the desert and ghandarva vile (where legit no one wears a terminal). So it doesn’t work outside of Sumeru
