Salford Together is a partnership of health and social care organisations working together. In Irlam and Cadishead, the teams are co-located in the same offices allowing them to work more closely together providing more personalised care.
Together they work with a range of other local organisations including the fire service, housing, learning disability services and AGE UK. Working in this way allows staff to look at complex cases from a multi-professional perspective and develop joined-up solutions with the patient at the heart of all decisions.
